Queensland Birth Certificate

You can apply for your child's birth certificate three weeks after the final Adoption Order is made.

You will receive a letter from the Department of Child Safety advising when the Adoption Order was made.

An application for the child's birth certificate can be made directly to the Registry of Births, Deaths and Marriages at 110 George Street, Brisbane City or by email at bdm-mail@justice.qld.gov.au - bdm-mail@justice.qld.gov.au

You can also download the application from the Department of Justice and Attorney-General website.

There are fees for a birth certificate or for a commemorative birth certificate.
