Case plan
What is a case plan?
- it's a plan about you and how your life is going to be while you are in care; it may change at different times depending on what's going on in your life
- it's about what has to happen to meet your goals, needs and to keep you safe.
How do I get a case plan?
- a meeting called a family group meeting (FGM) is organised by your child safety officer (CSO)
- your case plan is worked out at this meeting.
Who may be at my case planning meeting?
- you can attend so that people know your thoughts and feelings
- a support person, like a youth worker, can come with you if you like
- people in your life - like your family, carer, youth worker, school staff, CSO, and people from other services who can help you
- if you are from an Aboriginal or Torres Strait Islander background, someone from an Indigenous service may be there to support you.
What info may be in my case plan?
- your goals and needs
- what you need help with
- how contact between you and your parents will work
- how your cultural identity can be supported
- how your health and education needs can be met
- any other info about how to support you to meet your goals and needs
- info about what your parents need to do to help keep you safe.
Who gets a copy of my case plan?
- you
- your parents
- anyone else involved in your case plan will get a copy.
When is my case plan reviewed?
- at least every six months.
Why is it reviewed?
- to see how things are going and if anything needs to be changed in your case plan
- so new information can be added to your case plan.
How can I be involved?
- you can go to the meeting to talk about your goals and needs
- you can write down your thoughts and feelings and have someone read it for you at the meeting
- if you don't want to go you can tell people like your CSO, carer or counsellor everything that you would like said about your goals and needs and how you'd like your life to be for the next period of time
