Child health passport (CHP)
What is a CHP?
Your CHP is a folder that has information in it about your health.
What is it used for?
- to give your carer information about your health so they can help to meet your health needs
- to help the department and your parents (if this is okay) understand your health and help you to be healthy.
How do I get a CHP?
- your carer will take you to your doctor for a check-up
- when you visit the doctor, they may suggest that you see other types of doctors (like a dentist or an eye doctor) to help you be healthy
- all the information from your visit to your doctor is put in your white CHP folder.
What happens then?
You visit your doctor at least once a year for a check-up and any new info about your health will be added to your CHP folder.
Who looks after my CHP folder?
Your CHP is given to your carer.
How can I be involved?
- if you want to see your CHP ask your child safety officer (CSO) or carer
- if you think the CHP is missing any important information about your health tell your carer or CSO
- if you move placements remind your carer that your CHP should come with you to your new placement
- to find out more about CHPs you can talk to your:
- CSO
- carer
- youth worker
- community visitor.
