Purpose
The purpose of this funding is to enable non-government organisations to deliver support services that meet the needs of young people aged 15 to 17 years who are preparing to leave care.
Target group
The client group for services are:
- young people aged 15-17 years subject to child protection statutory intervention and requiring support to transition from an out-of-home care placement (specifically Foster and Kinship Care, Specialist Foster Care, Residential Care, and Supported Independent Living);
- young people aged 15-17 years who have removed themselves from an out-of-home care placement prior to turning 18 and require transition support services; and
- young people aged 18 years where a Support Service case has been opened to facilitate up to 12 months support (as assessed by the Department) on unresolved transition from care issues.
