Blue cards
All foster carers, kinship carers and their adult household members as well as certain persons associated with licensed care services (persons in regulated employment) are required to hold a blue card issued by the Commission for Children and Young People and Child Guardian (CCYPCG).

Licensed care services
Nominees or directors
To be completed by people who are, or are applying to be, a nominee (paid or volunteer) or director of a licensed care service and returned to a Child Safety Services zonal office:
- Nominee or director of a licensed care service blue card application form (LCS/B)
- Nominee or director of a licensed care service blue card renewal form (LCS/B renewal)
Authorisation forms are used when the applicant already holds a current blue card. It permits CCYPCG to inform the department of the current status of an application, the validity of a blue card and any changes to the status of an application.
Paid employees
To be completed by paid employees of a licensed care service and returned to the CCYPCG:
- Paid employee of a licensed care service blue card application form (LCS/P)
- Paid employee of a licensed care service blue card renewal form (LCS/P renewal)
For paid employee authorisations, use the 'Authorisation to confirm a valid blue card' form, available from the CCYPCG website.
