Ongoing intervention phase (Permanency planning)

Our department aims to ensure that a child is in a stable care environment and endeavours to safely reunify the child and family.
Where children cannot be safely reunified with their families, other permanency arrangements are considered. This includes taking out a long-term child protection order and ensuring that the child is provided with a stable, out-of-home care environment until they turn 18 years.
Planning for the young person's transition from care commences from the age of 15 and may involve the use of a support service case to provide additional assistance for their transition to independence following their 18th birthday.
In a small number of cases, birth parents seek a secure and permanent family for their child through adoption.
